Make your request at least two weeks before the print or event deadline. Requests can be made by this online form. Please include all necessary information in your request:

• Your contact information (phone, email, mailing address)

• The date of the event

• The date you wish printed on the citation

• The name of the individual(s) or organization being honored

• The reason for recognition (1-2 sentences maximum)
